PlanWell (decision-support guide)
Description:
The PlanWell website can be used to help patients better understand the different types of medical care, and what would be important to them during serious illness. They can then use the “Make a Plan” tab to document their values and preferences, and to print out a “Dear Doctor” letter that can be shared with their health care providers and loved ones. A recent randomized controlled trial using the resource showed positive impacts from both patients and family physicians (publication pending).
